We currently operate out of two locations: our main office in Winston-Salem and our satellite office in Mount Airy. Maps to both locations can be found on our contact us page here. When you come for your visit, please bring the following: any medical information (medications, past medical history, primary care physician), dental/medical insurance information, and any X-rays or photographs from your referring doctor. In order to serve our patients efficiently, we see patients by appointment only. We make every effort to see our patients on schedule, however, emergencies may arise that require our immediate attention. We will notify you of any delays and make arrangements to lessen your inconvenience.

At this time, we do not participate with any insurance plans, but we will be glad to file your insurance claim for you. All fees are based on the complexity and the time required to complete your evaluation or surgical procedure. We do ask that you pay at the time service is rendered any amounts we estimate to be not covered by insurance. For your convenience, we accept Visa, MasterCard, personal checks, and cash.
